The Greirat quest is such a buggy piece of garbage. This is the second playthrough that has randomly had him die on me when he should've lived. Here's what I did: Rescued Siegward from the well after beating the Cathedral. Sent Greirat off to Irithyll. Beat the Old Demon King.. Jordans 90

Greirat Missing after first mission. I freed Greirat after killing all the lords and sent him to pillage, he has not returned from the undead settlement and I am worried. However, a boss needs to die in your world in a certain amount of time after sending Greirat or he dies.
